Fiz o 1, 2 e 3 com o /etc/init.d/squid stop e start e depois o reconfigure
4. Verifique os logs de configuração atualizada com:

O cache.log continua com o DEBUG: sgFindUser called with: nome_de_usuario
No db, eu adicionei um dominio, gameforge.com e usei o comando squidGuard -C /var/lib/squidguard/db/gamesonline/domains && squidGuard -u && squid -k reconfigure
E aí começou tudo isso.
Acabei de notar que nos sub-processos do squid tem isso aqui...
23789 proxy 17:37 (squidGuard) -c /etc/squidguard/squidGuard.conf
e tem cerca de uns 129 deles.
